Format

You will review the patient case that you managed in assessment task 1.

Part 1: 700 words. You will prepare a handover using the template provided on Blackboard. The timeline for the handover is at the point of transfer from your ward to a high dependency unit (required because of the patient's deterioration in health status).

Part 2: 500 words. You will write a written reflection. Using the template and the CICO tool to review your practice from task 1 and to incorporate the feedback you received. Based on your reflection, set out your plan for the next steps in your learning.
