Create a plan for how to conduct a root cause analysis to investigate this problem. Also, prepare a memo to the Operations Manager detailing how the steps in the article could be utilized in the organization to improve quality.

The student is to create the root cause analysis plan and memo based on knowledge learned in the scenario, and post it in the discussion.
