Question

A possible mishandling of cultural differences that can occur in the clinical setting involves a patient that is a Jehovah's Witness who requires surgery to repair a broken hip after falling at home. In this scenario, the patient has an elevated INR that needs addressing prior to any surgery. Due to significant pain, complete lack of mobility, the need for surgical repair is urgent. Typical treatment would include fresh frozen plasma transfusions. Cultural mishandling happens in this case, when the patient's health care providers determine that Jehovah's Witnesses typically refuse blood products and transfusions, which includes FFP. Delaying treatment results in delays in care. Healthcare providers need to inquire at a deeper level what treatments the patient is willing to accept. Other options that may help stabalize the patient for surgery need to be discussed and explored with the patient.
