Calian Lewis is an 84-year-old male who has a PMH of glaucoma, hypertension, CVA with left sided deficits requiring the use of a walker, as well as difficulty with ADL's. Elu Lewis, the wife of Calian, is a 74-year-old female with a PMH of osteoarthritis, IBS and hypothyroidism. She does not have any mobility issues and does all the cooking and other household chores. They live in a beautiful single-family home in the country on 20 acres. They do not have any children or family members nearby and neither of them drive. They are worried about their deteriorating health and are feeling isolated. They have saved for retirement and are planning to sell their home.
