Question: You are not creating your own audiogram data; rather, you will use an audiogram from google and plot that audiogram out on the form to submit. Do not copy and paste an image of an audiogram. Plot the audiogram yourself using data from your resource. Please submit the audiogram link from google.

Values for the acoustic reflex and the OAE spaces may be entered as simply P (present) or A (absent); actual numeric values are beyond the scope of this class. Likewise, tympanogram type will be sufficient; numeric values will not be needed. Unless there is a reason stated in the narrative, pure tone averages (PTA) and speech recognition or awareness thresholds (SRT or SAT) should be included. Word recognition scores may be noted as simply "good" or "poor."

Remember to plot air and bone conduction thresholds. Include proper masked symbols for thresholds obtained with masking. Remember that thresholds obtained from sound field testing such as VRA will be plotted as "S" rather than "X" or "O."
