Emergency Department Notes Vital Signs Allergies and Medications 9/17 1130 The client fell onto the right wrist and forearm. The client was able to move the arm but stated, "It hurts to move it." The right arm was not broken, but it will need support and restricted movement. The arm was cleaned and revealed a 5-inch (12.5-cm) long abrasion on the right hand, wrist, and forearm with broken skin and redness. No stitches were required, but the abrasion was wrapped. 9/18 1245 The client is a 35-year-old male seen 24 hours ago for right arm abrasions after falling from a pick-up truck to a gravel-filled dirt road. The client was discharged after cleaning and immobilizing the right hand, wrist, and forearm. Client was given discharge instructions for cleaning and immobilizing the arm. The client has returned to the emergency department, stating that the right arm pain has gotten worse (rates it a 10/10), and that "something yellow" is oozing out of the bandage. The client refused to clean it, stating, "It hurts too.
