Clinical Case Study Assignment
Overview
This assignment requires you to conduct a case study for a patient who attends the pharmacy or is a patient identified from the site where you do your placement. Choose a patient if possible who is taking at least FIVE different medications. At least ONE of these medications should be used in treatment of the conditions studied in PHAR3101, PHAR3102 or PHAR3103 this semester. The patient should ideally have some sort of presenting complaint/reason that prompts the review and should not be a routine medication review or Meds Check.
The patient case study is presented in THREE parts.
Section 1: Detail what further information would assist in making your assessment of this patient. Information could be obtained from the patient, diagnostic or laboratory tests. Explain reasons for obtaining this information.
Section 2: Based on the information you have gathered, identify any potential and actual medication-related and disease-related problems.
Section 3: Using S.O.A.P template, write a letter outlining your findings and recommendations to the patient's GP.
You should ask the preceptor for guidance in identifying a patient, and any further information about the patient that may be valuable to your case. You should do this as early as possible in your placement. If feasible, interview the patient to gain a complete picture necessary for conducting a medication review. If it is not possible to interview the patient, discuss alternatives such as role-playing with your preceptor. Patient histories within the pharmacies' dispense program and patient packing profiles for dose administration aids can also be used to obtain current medication's and recent changes the patient may have undergone. For hospital placements, patient charts and notes may be used to obtain information.
