Assessment task - Professional Practice Assessment
Professional practice assessment task 1:
As part of professional practice assessment task 1, you should perform the following activities on three (3) wounds in your workplace:
- Perform wound assessment.
- Refer to the workplace documentation relating to wounds such as wound care assessments and treatment chart.
- Provide health education to the person, family or carer appropriate to the situation.
- Perform appropriate wound care such as wound cleansing and wound dressing. Ensure you appropriately modify wound management strategies, procedures and goals for the individual person in accordance with their treatment chart.
- Evaluate the person's wound care.
- Complete relevant workplace documentation.
You must work under the supervision of a registered nurse complying with your workplace policies and procedures underpinning wound care management such as infection control policies and procedures.
The wound care management you perform must involve a simple wound dressing and at least one (1) must involve a complex wound dressing.
Simple wound dressing may involve dressing for:
• Bruises
• Minor cuts
• Lacerations
Complex wound dressing may involve dressing for:
• Surgical wounds including managing sutures, staples or drains
• Diabetic ulcers
• Acutely discharging wounds
• Burns
• Necrotised wounds
• Pressure ulcers
• Chronic wounds related to vasculitis etcetera.
You are required to access a range of wound care resources to provide appropriate wound care management to each person. Examples of these resources include, but are not limited to, the following:
- Appropriate wound cleaning solutions such as warmed saline.
- Appropriate primary dressing materials such as synthetic fibre gauze, island dressing, hydrocolloid, silicone dressing etcetera.
- Appropriate secondary dressing materials such as semipermeable film dressing, non-adherent dressing, foam, gauze, highly absorbent pad, combine, hydrocolloid etcetera.
- Appropriate fixation materials such as paper tape, tubular bandage, mefix etcetera.
Complete the following template for each wound care management activity you performed. You must familiarise yourself with the observable behaviours identified in the assessment checklist for each activity (provided in the logbook). The Host Facilitator or EQUALS' Clinical Facilitator will benchmark your performance with these observable behaviours and complete the checklist and outcome section following your wound care management.
