You will find two documents attached in this email one is a report in that topic we want the same topics in our report, and in the other document is an excel sheet that shows results of answers from seven people answered a questionnaire. So these two is related because in the report you have to talk about the answers you have got as it shows in the example report attached. You don't have to write a new questionnaire you will find photos of a questionnaire that you can use it and write different answers. About the picture this is a survey that i mention in the email which is needed to submit for 7 people and then the result be mentioned in the report
