1. A nurse is busy and pulls meds for all 4 of her patients at once. The nurse inadvertently gives a BP medication to Mr. Jones when it was actually ordered for Mr. Smith.
2. A nurse misreads the MAR and administers an antibiotic IV instead of IM. As a result, the patient suffers cardiac arrest.
3. A nurse pulls Tylenol to give to her patient with a headache. The order is for 650mg. Usually the unit stocks 325mg tablets in the med cart. The nurse fails to notice the drawer was stocked with 500mg Tablets and administers 2 (1000mg).
4. A nurse administers a BP medication to her client. 45min later the client gets out of bed and falls because he didn't realize the medication could cause dizziness.
5. A nurse gets busy and forgets to chart the PRN morphine she gave to Mr. Smith right before shift change. He continues to complain of pain, so the nurse taking over gives another dose of Morphine. Patient experiences respiratory depression and has to be given Naloxone (Narcan) to stabilize him.

The unit is loud and the nurse misinterprets what the provider says and writes the order for Inderal (Propranolol), an antihypertensive medication. The patent experiences severe hypotension as a result.
9. A nurse misreads the MAR and gives a medication at 0100 that was actually scheduled for 0700.
10. A nurse gets busy and asks another nurse to administer her patient's pain medication. The other nurse gets distracted and forgets. The patient and his family complain to administration that he waited several hours for the pain relief.
