A 80 kg (176 lbs) patient suffered a cardiorespiratory arrest two weeks ago. Since that time the patient has had an endotracheal tube in place and has been in assist-control mode at a Vt of 700 ml, rate 12 bpm, and a FiO20.35. His ABG shows pH 7.37, PaCO2 43, PaO2 85. NIP is -5 cmH2O and VD/Vt is 40%, Spontaneous Vt 200 ml and spontaneous respirations are seven per minute. The therapist should recommend with of the following at this time?

No change in the patient's ventilator parameters at this time Decrease the patient's Vt to 600 ml Change the patient to an SIMV rate of 7 per minute Discontinue mechanical ventilation and extubate.
