Question

Patient Reason Physician note Assessment/ Procedural Code Frances p/u completed Sun Life STD forms. Patient had knee replacement surgery 3 weeks ago and will not be able to return to work for 3 more months. Physio to begin next week. Patient to continue with Tylenol extra strength 500 mg and alternate with Advil extra strength 400 mg as needed for pain. May need walking assistance until physio strengthens the muscles. Patient may return to work after a further assessment in 3/12.

Bradly 6-weeks post broken leg skiing - cast removal Intermediate Assessment performed. Patient's femur has healed well. Take it easy for the next couple of weeks, continue to use crutches, physio referral and flu shot given.

Bent Having difficulty hearing. Intermediate Assessment performed. Recently released from the military. He was a Marine Technician - SN C60 987 234. His medical release indicated his hearing was within normal range. However, since retiring he has noticed more difficulty hearing. After completing a hearing test, he will require hearing aids. A referral to the audiologist at University Hospital. This is covered under the veteran's program.
