A patient presents with complaints of periumbilical pain that started as colicky and then localized to RLQ with onset of 8 hours ago. Yesterday, she noted decreased appetite and some nausea with vomiting. No change in bowel habits, denies constipation or diarrhea. During the physical examination, the patient has rebound and involuntary guarding at one third of the way between the anterior superior iliac spine and the umbilicus in the right lower quadrant.
