Question

The patient presented for follow up medication management. He presents with his discharge summarry from UMMS which shows his previous admission at St. Joseph bhospital was alcohol ABUSE.He is accompanied by his niece who is his choosen interpreter and he agreed to her being present and interprating. Through his niece, he reports ongoing migrains. He reports medication compliance and denies any side effects from his medications. He reports his mood, sleep and energy level as adequate.He denies SI/SA/H/AH/VH.He denies concurrent drug use.He reports his last drink was 4 days ago over the weekend. The session also covered the importance of maintaining a balanced diet, regular physical activity, sufficient rest, keeping appointments, and adhering to prescribed medications for overall mental health. The patient acknowledged understanding these points. Additional support and psycho-education were provided during the session.
