Question

Project for A patient in the ICU is very sick after a Myocardial Infarction. The patient is placed on a heparin drip, but the nurse incorrectly calculates the dose, giving the patient 3x the regular dosage. It likely would have been caught had she performed the required check with an additional nurse prior starting the drip. The patient begins hemorrhaging and passes away.
