The patient is experiencing numerous symptoms associated with MDD including mood changes, reduced executive functioning and ability to concentrate. The patient is also experiencing somatic symptoms such as fatigue, headaches, altered sleep patterns, changes in appetite and changes in weight. The patient's symptoms suggest a moderate to severe episode of depression given the extent of how these impacted her ability to function and work. The patient's symptoms also seem to be impacting the relationship with her children which may be causing a sense of guilt. What classifications of medications can be used to treat this disorder? Which medication do you recommend, and why? I would start with a low dose SSRI such as Lexapro or Zoloft. I would start the patient on the lowest dose for the first 1-2 weeks and then work up to the therapeutic dose as tolerated. These medications have a relatively safe profile. Probiotics as an adjunct treatment has also been found to help alleviate MDD symptoms I would also recommend the patient a Magnesium supplement the patient can take before bed to assist with sleep symptoms.
