During resident data collection her daughter states she was walking short distances with a walker up until her fall. She also tells you she usually has a great appetite but, in the hospital, she didn't eat much and attributes it to the ground diet she was on there. She reports she has been living on a yogurt and maybe a bowl of soup a day for the past couple of days. She tells you that she was in bed for the most part of her hospital stay and feels she would eat better if she was out of bed and assisted with cutting her food. She has had a weight loss over the last 6 months and refuses to wear her upper denture. She also tells you she has been incontinent for many years due to her advancing dementia and wears an adult diaper at all times.
