Mr. Harding has been mostly in his dorm room except for meals. On approach he was quiet, preoccupied and selectively mute. At times he was observed self dialoging and responding to internal stimuli. He refused vital signs assessment. Nursing staff will continue to monitor his throughout this shift for safety and needs.
