Make an ISBAR Hand over and progress Notes for the following patient. Patient complained of sore throat, having husky voice and also a lump in his left leg. Requested to see Dr. burger for fortnightly check-up. Incorporate you're a-g assessment. Dr. Reviewed and prescribe inflammatory medications. For the lump dr. suggested to get it tested but patient refused as he is terrified to have radiations again. Involve family and social worker to help patient in decision making.
