She reports intermittently noticing yellow discharge draining from her bottom left tooth, and denies any pain. She states the last time she went to a community health dental clinic, she was told she needed prophylactic antibiotics before teeth removal. Prior to this visit, she has refused all anti-diabetic medications related to the loss of her brother due to kidney failure, which she believes is related to the metformin he was taking. Discussed diabetes complications at length with patient and the risks involved in continuing with uncontrolled blood sugars. She states she periodically checks her blood sugar and states they are "very elevated". She is currently experiencing additional symptoms of uncontrolled diabetes including blurred vision, and yeast infections in her skin folds. Today, she is agreeable to having a pap smear and routine bloodwork completed.
