Mr Kim Liu (MRN 727991) is a 28 year old Korean man who was brought in by paramedics after a motor vehicle accident. He doesn't speak any English. He suffered an open fracture of the left tibia and fibula. He has left upper quadrant pain, and a seatbelt mark along his left shoulder, neck and chest. Through an interpreter it is determined that he has no known allergies and he has significant pain-9 out of 10-despite intravenous morphine given by the paramedic on site. His observations are as follows: Temperature-37.0°C Heart-rate= 100 Respiratory-rate=28 breaths/per minute BLOOd pressure-150/86 mmHg O2 saturation= 93% (6 Litres via HM) A chest X-ray suggests some stable rib fractures and leg X-rays reveal crush injuries and comminuted fractures of his left tibia and fibula. An abdominal CT demonstrated a splanchnic haematoma. Mr Liu is going to theatre shortly for an open reduction and internal fixation (ORIF) of his leg fractures and, depending on his haemodynamic stability, he may also have a laparotomy for the splanchnic haemorrhage.
