Question

The nurse in the outpatient urology clinic is caring for an older adult client. Nurse's Notes Client presents for evaluation of concerning urinary tract symptoms. In the prior nine months, the client experienced intermittent, painless hematuria and completed antibiotic treatment for two persistent urinary tract infections. Urine specimens for cytology have shown atypical cells. Client's history includes well-controlled essential hypertension and osteoarthritis that is managed with over-the-counter analgesics. Client has no food or medication allergies. Negative history for renal disorders. Computed tomography (CT) urogram with intravenous radiopaque contrast media is planned. The nurse reviews the client's history, documents nurse's notes, and performs an assessment to prepare the client for the urologic procedure.
