You are an outpatient coder in Health Information Management. Every Friday, a volunteer works in your department, preparing ER records for scanning. The volunteer has helped in the department for many years, and is well respected and liked by everyone. Passing by her desk one Friday, you cannot help but overhear her telephone conversation. "Oh Alice," she says, "I couldn't believe it! Martha Owens was in the ER five days ago having an abortion! I never knew she was even pregnant, did you?" The volunteer pauses, obviously listening to Alice's reply, then says, "well, I bet her husband doesn't know either!" You move away, but later you end up coding Martha Owens' ER record. The diagnosis is "missed abortion" When you go to your supervisor to let her know what has happened, she tells you she will handle it. Even though you expect your supervisor to meet with the volunteer next Friday, no meeting occurs that you know about. The volunteer still comes into the department to work as usual.
