PART - PERSON CENTERED CARE EXERCISE
This exercise will assist you to recognise the requirements of Person Centred Care and support the resident/ client's social self. It is designed to link to the resident/ client's care goals and build on their identified strengths. You must involve those people who care for the client on a day to day basis as well as (if appropriate) family members when negotiating this exercise
Exercise
You are required to consider the goals and strengths of three resident/ client's within the workplace. One of these resident/ clients must have a diagnosis of dementia and one must be from a Non English speaking background. During this selection process discuss the outcomes of this exercise with your mentor and/or supervisor to assist you to select the most appropriate resident/ clients.
Once you have done this you will need to:
- Speak with the resident / representative to gain their acceptance for participation in the project.
- Speak with the Supervisor/ mentor to gain access to the care plan and discuss your ideas regarding activities that may enhance the residents person centred care (this can be story sharing, hands on activities and / or support, craft, etc) use your knowledge of their history, strengths and life style preferences and your imagination to identify an activity.
- These activities must be undertaken on a one to one basis and enhance the quality of care for the resident/ client, building on their strengths and preferences. Feedback regarding the acceptance and progression of the interaction must take place with the supervisor/ mentor.
- Design an activity that you will be able to do 1:1 with this resident based on their interests and strengths, recognising strengths based solutions that may be necessary to respond to routine and or unpredictable problems related to the care relationship (outline same in response box)
Document your discussion with the Supervisor/ mentor including the activity to be undertaken, proposed inputs and outcomes based on the resident/ client goals and preferences.
Implement the activity and answer the following

1. Provide a brief case study relating to information collated to support the resident/ client 's inclusion in this project. Outline barriers to participation or engagement. Include the identified goals and objectives and proposed outcomes.
2. Outline the intervention and overview the benefits of the exercise for the resident including any risks that were considered
3. What changes, if any, did you observe during this exercise? Identify changes, you have observed in behaviour/ emotional/ other responses of the resident/ client during this time (include negative and positive reactions and your responses) - use the attached response template
4. How did you feel during the design of this activity for the resident/ client and at the completion of the activity, including any changes in your attitude or understanding of the person centred needs of the resident / client?
5. What feedback did you receive from the resident/ client or other stakeholders regarding resident/ client s feelings during the activity? What did you do with this information?
6. Did you meet the proposed outcomes? If not, what challenges, behaviours and barriers did you encounter? If yes what strategies would you recommend to incorporate this activity into the resident/ client s daily routine?
OVERVIEW STATEMENTS

Overview three situations relevant to legal and ethical practices within the workplace and any improvements that should be considered
Identify three situations where you have supported, reported and documented resident/clients palliative needs (these can link to dates in your journal)
Identify three occasions where you have supported the healthy functioning of human body (this can link to dates in your journal) What did you do?
Overview a workplace WHS meeting (day meeting, handover, staff meeting or any meeting safety and or wellbeing) or inspection that you have attended and participated in demonstrating how you contributed to the meeting or inspection.

Consider your thoughts about the diverse workplace - the people you are meeting, their demands and allowances that are made for different groups, cultures and demographics and reflect on how you feel about this- you must document your thoughts openly and honestly
