Mr. Potter is a 68-year-old male recovering on an orthopedic floor from a total hip replacement. He has a past medical history of type II diabetes, dyslipidemia, and hypertension and previous CABG. He currently takes Metformin, lisinopril, aspirin, and atorvastatin. You have been called because his urine output has consistently been less than 20 mL per hour since the operation, 18 hours ago. The operative note states that the surgery resulted in significant blood loss and estimate that he lost 2 L of blood. He required vasopressors at numerous points during the operation to keep his blood pressure above 90 mmHg systolic and he received four units of blood during the operation, plus a further 2 L of lactated ringer solution over the last 16 hours on the orthopedic floor. Mr. Potter looks pale and lethargic, a urinary catheter in place has been flushed and is patent, he is not thirsty, but does feel mildly short of breath. His heart rate is 90 bpm and blood pressure is 150/ 80 mmHg. His JVP is visible at about 6 cm above the sternal angle and there are faint crackles at the bases of his lungs. He has no murmur or gallop.
