Patient had an acute MCA infarction/left sided weakness and has multiple falls and presents to the orthopedic floor for observation. Neurology consulted-Watchman CT noted no evidence of thrombus within the appendage. Small right Pulmonary Embolism- right lung. Duplex study of lower extremities shows acute partial deep vein thrombosis in the right femoral vein with abnormal flow. Evidence of acute partial DVT in the right popliteal vein with abnormal flow dynamics. SMART #1 Outcome Statement: Within 24 hours, patient will demonstrate improved tissue perfusion as evidenced by: Maintained peripheral pulses Normal capillary refill (<2 seconds) No new symptoms of DVT/PE Stable vital signs within normal parameters What measurable data did you use to determine whether the client met, partially met, or did not meet the SMART #1 outcome?
