You are a case manager RN working in the organ transplantation department of a large university hospital in a metropolitan area. The job of the case manager RN is to manage the care of patients who are awaiting transplant. After transplantation, the case manager RN then assumes the role of assisting the patient in maintaining optimal health to ensure long term success and health with the newly transplanted organ. During both the pre-and post-transplant time the case manager RN will be assisting the patient in setting up appointments, managing medications, monitoring laboratory values, and often a variety of other duties will arise as well. The case manager RN is an integral part of the transplant team and has a vital role to ensure success. In this situation, you are the case manager RN for a 35-year-old male with renal failure caused by uncontrolled type I diabetes (T1D). Current medical history includes: renal failure, anxiety, depression, hyperglycemia, neuropathy, and renal dialysis three (3) times a week. Social history includes: divorced, father of three (3) children, unemployed, no current source of income, insurance has lapsed and has no coverage, living with his parents who are healthy (ages 60 and 65), children live.
