Operative Report, Ethmoidectomy and Antrostomy In this surgical session, the surgeon performed three procedures, each of which is highlighted in the Procedure Performed section of the report. LOCATION: Outpatient, Hospital PATIENT: Inez Epley PHYSICIAN: Gregory Dawson, MD PREOPERATIVE DIAGNOSIS: Recurrent acute left-sided sinusitis and left conchal bullosa, left nasal obstruction POSTOPERATIVE DIAGNOSIS: Recurrent acute left-sided sinusitis and left conchal bullosa, left nasal obstruction PROCEDURE PERFORMED: Left functional endoscopic sinus surgery and removal of left conchal bullosa (this is a concha bullosa resection). The patient also had left anterior ethmoidectomy and left maxillary antrostomy. ANESTHESIA: General endotracheal anesthesia PROCEDURE IN DETAIL: Following informed consent from the patient, she was taken to the operating room and placed supine on the operating room table. The appropriate monitoring devices were placed on the patient, and general anesthesia was induced. She was orally intubated without difficulty. The left nasal cavity was packed with Afrin-soaked gauze. This was removed after 5 minutes. The patient was draped in the usual sterile fashion. The lateral nasal wall on the left-hand side was injected with approximately 2 cc of 1% Xylocaine with 1:100,000 units epinephrine. The widened left middle turbinate was also injected with 1 cc of the same.
