Operation: Hypospadias repair

Anesthesia: General with penile block

Procedure: An 11-month-old with glandular hypospadias. He has already been circumcised. Parents are requesting hypospadias repair, which is performed today without contraindications. The patient was brought to the operating room and placed supine on the procedure table, given general anesthetic and prepped and draped per protocol. Surgical verification and patient verification were performed. Intravenous antibiotics were administered and a penile block was administered using 9.5 mL of 0.25 percent Marcaine without epinephrine in order to assist with postoperative analgesia. Prolene suture was placed through the glans penis and the meatus was opened utilizing a hypospadias pickup. A deep back plate incision was made after a tourniquet was applied and the urothelium was then advanced to the epithelium of the glans with nterrupted 7-0 Vicryl sutures. The corners were then placed with small clamps for stay sutures and then the back plate was then incised thoroughly. The urethral muscosa was then advanced to the epithelim in the midline and then on either side with interrupted 7-0 Vicryl sutures such that the entire bride of tissue had been obliterated and the urethra was in the normal glandular position. Hemostasis was meticulous. The tourniquet was removed. Bacitracin was applied. The patient was then awakened and transferred to the recovery room in stable condition. There were no complications.
