A. Neck muscles symmetric and trachea is midline. Denies neck pain or stiffness. Left-sided neck weakness with muscle strength testing. Head drooping to the left noted after performing ROM. Left eyelid droop noted. Oriented x 3. Speech is clear.

B. States that for the past 3 months, their neck is weak and they have trouble holding their head up. Reports coughing more whenever they drink liquids.

C. Client is oriented. One eyelid is drooping. States they choke whenever eating. No abnormal findings noted on neck. Head is tilted after the exam. Talks okay.

D. I asked the client why they came to the clinic and they told me that they have some trouble holding their head up and cough a lot whenever they drink. I'm pretty sure its been going on for a while. When I looked at the client, I could see that the left eyelid was hanging lower than the right one but the rest of the face looked okay. I explained how to do all the exercises to test neck mobility and strength. I then watched them do them. The client did fine but then I think the neck was tired because their head was hanging to the left side a bit.
