Rephrase : The patient is presently being monitored every fifteen minutes and is also on a wing restriction. However this evening, she was seen alone on the B-wing pouch with a male peer, patient was previously educated that she should not be alone with male peers, patient was educated again about the situation but was irritated and argumantive. She was asked to go back to her wing and complied with the stated wing restriction. She was later seen watching TV in the communal areas of the unit. She refute suicidal ideation (SI) or self-injurious behavior (SIB) during this shift. The nursing staff will persist in vigilantly overseeing her safety and needs throughout this shift.
