The ED note states, "Per EMS, the patient was saturating at 60% on room air and was tachypneic." She was placed on CPAP and received IV furosemide, posteriorly She indicates that her symptoms improved, she has had no need or respiratory therapy.The patient has been admitted to GMF for probable new-onset heart failure and severe symptomatic anemia records , cardiology evaluation reviewed , plan discussed with resident. Seen examined at bedside : looks comfortable NAD . Daughter at bedside The progress note should reflex how you recive the patient on the start of the shift, during and end shift note.
