1600-Patient complains of numbness and tingling in the right hand, especially during activities involving gripping or repetitive movements. Patient reported pain radiating from the wrist to the forearm and occasionally up to the shoulder. The patients symptoms have been interfering with his ability to perform his job duties. Vital signs BP: 120/80mmHg HR: 78 BPM Resp: 16 breaths per minOxygen 98% Patients right hand is slightly swollen compared to the left hand. Administered acetaminophen 650 orally to relieve pain, as ordered by physician. Proceeded the patient with a wrist splint to immobilize the wrist and relieve pressure.
1800-Patient reported slight improvement in pain and tingling after using the wrist splint. Patient inquired about alternative treatment options. Administered a corticosteroid injection into the carpal tunnel, as ordered by the physician to reduce inflammation. Patient was scheduled for a nerve conduction study to confirm the diagnosis and assess the severity of the condition.
2300-Patient reported significance improvement in pain and tingling after the corticosteroid injection. Conducted a follow-up nerve conduction study to assess the effectiveness of intervention and confine the diagnosis of carpal tunnel syndrome. Educated patient to schedule follow up appointment with the physician in two weeks to evaluate the long0term effectiveness of the interventions.
