Question

To assess Mrs. Peterson's skin situation , the nurse would check the area for more redness, excoriation, or other signs of skin breakdown. She would also assess Mrs. Peterson's pain level and mobility. To evaluate the diagnosis of self-care deficit, the nurse would assess Mrs. Peterson's ability to perform activities of daily living, such as bathing, dressing, and eating. She would also assess Mrs. Peterson's level of fatigue and her ability to ambulate. The nurse would assess Mrs. Peterson's cognitive functioning and level of confusion. The nurse would reassess Mrs. Peterson's skin integrity and self-care deficits every 4 hours. Reassessments of Mrs. Peterson's skin integrity problem would include an assessment of the size, depth, and location of the skin injury; an assessment of Mrs. Peterson's pain level; an assessment of Mrs. Peterson's skin turgor; an assessment of Mrs. Peterson's mobility; and an assessment of Mrs. Peterson's cognitive functioning.
