Case Study for Supplementary Assessment Task
Situation: Peter Dawson is a 32-year-old single male who was diagnosed with Crohn's disease in his early 20's. Until recently he has been well and healthy and managing any acute exacerbations or "flareups" of his condition on his own. Unfortunately, Peter as a result of severe and acute exacerbation of his symptoms, now has a newly formed temporary colostomy (scheduled for reversal in 6 months).
Background: Peter has limited family support and lives alone and is working from home while he recovers from surgery (the surgical wound has closed and healed nicely with internal dissolving sutures). Peter can perform basic ADLs and is independently mobile. Peter is still learning to manage his stoma care. He says he is "in shock and didn't think his health would get worse so early in his life". His pain is managed but remains at a 3-4/10 for most days. He also stated he has suffered with abdominal distention, bloating and gas which is embarrassing. (Peter was 177cm tall and 57 kg on admission).
Other Previous Social History: Peter was working from home as a software programmer and likes to game online with friends and stay up late at night. His close family live in Queensland.
Medications: Flagyl 400mg orally TDS for one more week, Amoxicillin 500mg orally TDS for one more week, Humira 40mg once a week Subcutaneous injection, Imuran 50mg orally daily, Prednisolone 5mg orally daily, Panadol 1g orally QID for pain, Endone 5mg orally QID PRN as required for pain and diarrhoea and, Peppermint oil capsules orally 1 tablet before meals.
Instructions: Alongside his GP and Gastroenterologist, you have been referred Peter's case. As his Nurse Navigator/Clinical Nurse Specialist you are required to identify the current priorities of Peter and develop a Self-Management Plan for the next three (3) months with him to aid his recovery and rehabilitation at home. The plan is to review him in outpatient's appointment in 4 weeks' time and through telephone contact periodically.
