Assessment Task - Clinical Case Study
Task description: Drawing on the case study below, use relevant scientific evidence to critically analyse the situation, then describe an approach to assessment linked to underlying pathophysiology and develop and generate an evidence-based plan for care for your chosen problem (see list below) . After reading the case study choose one priority problem and respond to questions A to E below supporting your response with relevant evidence.
CASE STUDY - Jim Rovers
Jim Rovers is a 78-year-old man who lives at QUT's Happy Valley Aged Care Facility in Brisbane. You, a QUT graduate nurse, enter Jim's room and find him in his chair as is shown in the picture below (Picture 1).
Jim has a medical history of a cerebrovascular accident (CVA) in 2015 causing left sided weakness and was diagnosed with vascular dementia four years ago. Due to the CVA, he experiences problems with swallowing, dysphagia. His dementia has progressed to mid-stage (stage 5 on the Global Deterioration Scale (CGS) / Reisberg Scale) and he now experiences difficulties with his memory, reasoning, and communication. He also has osteoarthritis which causes him pain in his knees, hips and back. When you assisted him with his activities of daily living (ADL's) yesterday and the day before, he did not look this ‘uncomfortable/concerned'

After choosing your priority problem, critically analyse current, peer reviewed and relevant literature to support your responses to the following tasks:
A. Identification and Interpretation of Priority Assessment: Relevant to your chosen problem, describe and justify two (2) nursing assessments that should be performed as a priority to assess Jim's current health status.
B. Linking your chosen problem to underlying pathophysiology: Identify two (2) factors that could have contributed to the chosen problem and explain these factors in relation to underlying pathophysiology.
C. Development of Goal for Care using a SMART Goal framework: From the position of your chosen priority problem, and using the SMART framework, write a goal of care to address the chosen problem.
D. Evidence-Based and Person-Centred Interventions to Address the Goal for Care
Choose and justify two (2) evidence-based interventions to address the goal of care (at least one of these must be nurse initiated).
E. Evaluation of the Efficacy of Interventions
Discuss and justify how a nurse would evaluate the efficacy of the two interventions and identify the expected positive changes in assessment data.
