Question
Mary, Tom and Jack have recently become your patients. You have not seen any of them for the new patient consultation. Mary called your clinic today complaining that Jack, their 6-month-old son, has been having fevers, more fussy than usual, and seems to be playing with his right ear a lot for the last 4 days. He has been eating well so she did not think to call earlier, but now his fever is increasing. She does not have a home thermometer but states that she knows when her child has a fever. She states, "his forehead is burning up". Mary has not given him anything as she thought it would just go away on its own. Now that it has not gone away, she wants Jack to feel better and believes he needs antibiotics. She does mention he has some congestion and a mild cough for about 2-weeks. You directed your clinic to make a same day appointment for Jack to see you in your primary care clinic. Prior to your visit with Jack and Mary you reviewed Jack's old medical records from Nevada. Chief Complaint: Fever and increased fussiness.
