Poor care coordination has led to fragmented and disjointed care, leading to many problems in the healthcare system. These problems are high healthcare costs, medication errors, and poor healthcare outcomes. If this gap is not addressed, healthcare outcomes for the affected patients will worsen. Besides, the affected parties will suffer higher healthcare costs, leading them to sell their properties to pay for their healthcare services. Above all, this issue can contribute to the increase in healthcare disparities, particularly for those who are socioeconomically challenged.
