Question

A 20 year old male was involved in a motor vehicle accident. He is Rh(D) negative, had a negative antibody screen and received 68 RBC units, the majority of which were Rh(D) positive. The patient eventually recovered but has developed 6 clinically significant antibodies including Anti-D, C, E, K, S and Jkb. Two years later, the patient donated two units of autologous blood which were kept frozen for 8 years. The patient is now admitted for gastrointestinal bleeding and both of his autologous units were thawed and deglycerolized in preparation of possible transfusion. With fluid resuscitation, however, the patient stabilized. His hemoglobin is now 10 g/dL and his hematocrit is 31%.
