Question

Scott is a 57-year-old male presenting with neck pain and stiffness and lower back pain and stiffness. The lower back pain is mainly in the upper lumbar region but also in the lumbo-sacral region. Scott has a history of prostate cancer and had a total prostatectomy performed four years ago. Scott has been seeing a chiropractor for the past 8 weeks. The chiropractor had taken images in his clinic including both a cervical and lumbar spine series. The chiropractor proceeded to manage him with spinal manipulative therapy and heat therapy. He found that the lower back manipulation, where he was lying on his side, increased the pain in his lower lumbar spine, this pain has since nearly returned to the level of pain he was experiencing prior to commencing treatment. The neck and upper lumbar spinal pain he originally presented to the chiropractor with have not resolved. Due to his lack of improvement, Scott has decided to consult you as you have helped his wife in the past. Scott has the following signs and symptoms: Mild intermittent lower lumbar region spinal pain Moderate intermittent mid-upper lumbar spinal pain and stiffness Cervical pain that wakes him at night with associated cervical.
