Moving to a Value-Based Model Improved care and decreased costs are the goals of VBC. For vulnerable patient populations such as Paducah Health Medical System (PMHS) and patients in communities that are overlooked, VBC provides a network of support to promote health wellness and address risks. By focusing on the individual, this care model can help Paducah Health Medical System achieve better outcomes and reduce repeat ED visits. Using a proactive approach to healthcare, Paducah Health Medical System can provide care that reduces sickness and encourages health to the individual, which means better health for the overall population (eHealth Initiative, 2017).
