He sustained broken ribs and a hemothorax, which was treated with a chest tube placed in the emergency department. CT of head was negative, small laceration on his chin and scrapes all over his body. Blood alcohol level was 0.05 and positive for cannabinoids. He smokes 1 pack daily and drinks 6 pack of beer daily. Upon assessment his pain level was 6 out of 10, he was having difficulty pain control and morphine was administered. He begins to have hallucination and agitation. Lorazepam was administered. His blood pressure 161/93, heart rate 120, respiration rate 24, temperature 38.2, oxygen saturation level of 95% on 2 Liter of oxygen, and CIWA score of 17. Create SBAR for above scenario?
