Ms. Otero was visible throughout the common areas of the unit this shift. She was mostly observed in her dorm room laying on her bed-napping or pacing the unit hallways while intermittently self-dialoging, with bizarre hand gestures, facial expressions and mannerisms. She socialized and interacted with select peers and staff. Her dorm room remains untidy with clothes strewn all over the floor and tables. Nursing staff continue to encourage her to clean herself and room with moderate effect. She went outside for fresh air breaks and attended some group activities on and off the unit. She denied any SI/SIB- none noted or reported. No unsafe behaviors noted or reported. Nursing staff will continue to monitor her throughout this shift for safety and needs.
