• Mrs. Coutt- A 58-year-old patient was admitted to the hospital for laparoscopic appendicectomy. She is staying overnight as she is an insulin-dependent diabetic. Her blood sugars have been unstable.

• Sarah: A student nurse attending Mrs.Coutt Situation: Mrs. Coutt's friend has come to visit her. Both are laughing and reminiscing about an old memory. Sarah enters Mrs. Coutt's room to check her. She feels good to see Mrs. Coutt in a happy mood with her friend. However, Mrs. Coutt informs Sarah that she is experiencing severe pain in the area where she had her surgery and requests some morphine. Sarah is surprised by the request, as she had completed a pain assessment on Mrs. Coutt 20 minutes ago, during which Mrs. Coutt reported a pain score of only 2/10.
