You are a new HIM Director at a Community Hospital. Currently in your organization, pediatric patients are considered those that are under 13 years of age.

More information:You are aware sometimes the PEDS unit is used for overflow of adult patients. Also, not all pediatric patients are housed on the PEDS unit, examples - an older adolescent may be placed on the regular floor if they have an admitting physician that sees mostly adult patients so as not to make the physician go out of his way to the PEDS unit unnecessarily or critically ill pediatric patients may be placed in your ICU for stabilization and transferred from there directly to a Children's Hospital, so they would never be on the PEDS unit.

You just noticed the various criteria for reporting to your state pediatric disease registry are run at your organization based on the location of the patient unit of PEDS and the state's data dictionary for the pediatric disease registry supplies the definition of a pediatric patient as one that is 18 years of age or younger.

1. How will you design or modify reports based on the current data dictionary? (Create a database that includes the needs of the PEDS unit. The columns should include the titles of the columns). Describe the information that each column will hold.

2. Elaborate on what changes will need to be made going forward to collect the appropriate information for reporting? Review your newly constructed database and determine the steps the PEDS unit should do to gather the information.
