Maria is a 20 year old woman who crossed the Mexican- US border hoping for asylum. She was among thousands of such migrants bussed to NYC in recent months and reports she is staying with her partner, in a room in a family friend's apartment with several other people. She had been working in a local restaurant until recently, when her partner made her stop because she was "getting too friendly" with the other workers. She presents to the emergency department with the onset of preterm labor. She states that she is 6 months pregnant with her first baby and that she fell yesterday and cramps started a few hours later. In your assessment you note that she has several bruises on her face, back and her right arm and leg. When asked she vaguely responds that she slipped at home. You note she is very thin and reports having gained only 10 pounds. She keeps her eyes downcast and only speaks if prompted. What concerns do you have for Maria?
