John is a tenured employee and in the last ten years, he has received satisfactory, if not excellent ratings on his performance reviews. He has perfect attendance and is a very helpful employee. John never says "no" to any requests by his supervisors and always volunteers.

Auditors discovered the errors, and the company was cited and fined. Furthermore, John witnessed a safety issue, but didn't tell anyone about it. The employee who he witnessed was injured.
