Auden has had three acute care hospital stays in the last five years for bipolar II. They have been out of the hospital for 14 months now. They have a part-time job stocking products at a local grocer over night while it is closed and attend a clubhouse during the day 3 days a week. They struggle to stay focused and follow through on daily tasks. The social worker usually visits every other week, the tech visits three times a week, and the OT visits once a week. Each brings out their meds during the visits. There is some blurring of professional roles to support them. What type of CM is this?
