"Annual evaluations for all you employees must be submitted by Monday, June 1. Annual evaluations are required for all employees but are particularly important for employees on probation.

To complete the evaluations most efficiently, you should follow the procedures explained in the Employee Handbook. Rate their performance based on a scale of 5 (consistently exceeds requirements) to 0 (does not meet requirements). At the end of the form, list 3 strengths the employee brings to the job, and name 3 skills the employee can develop.

The procedure for annual evaluation is simply stated in the Employee Handbook on page nine. The procedure needs that you meet with the employee to show the evaluation and to obtain the employees signature on the form.
