Case Presentation In Japan pufferfish is served as a seasonal delicacy. Any chefs who want to prepare a meal using this fish has to go through training and licensed through the government. The reason for such precaution is because pufferfish has concentrated neurotoxin in the ovaries and liver, a smaller amount in the intestines and skin, and a trace amount in the muscle. It is considered to be one of the most poisonous vertebrates with no known antidote and had killed unknowing eater (average fatality rate is 6.8%). Even when prepared correctly, a pufferfish meal could cause numbing and tingling sensations. The neurotoxin is called tetrodotoxin (TTX) and its structure is shown below. It's been shown that it blocks voltage gated sodium channel.
